US: UPS Equipment Return Process

Returning Equipment to Medidata via UPS

If you are trying to return equipment to a Medidata office location, please click the “Label” button below and follow the steps outlined here to print your return label.

Step 1) After pressing the “Label” button, you will be redirected to the UPS portal, which will generate necessary labels.

On the next screen, please confirm CAPTCHA. Once completed, the website will redirect to the actual shipment form.

Step 2) This screen will present shipping choices. Former employees returning equipment should provide a personal email address in order to receive a shipping confirmation and tracking number.

Please select the office closest to your current residence to ensure the quickest turnaround time.

Step 3) Please select the equipment being sent. Proper selection will ensure proper insurance and pricing for the package.

Step 4) Please select your department.

Once completed, please select “Process Shipment”. This will bring up the shipping label, which is to be printed and securely affixed to the shipping box. UPS can be scheduled for pick-up, or the package can be dropped off at any UPS Store, Drop Box, or other authorized UPS Shipping Outlet.
