Study Suspension Restart Notification Form

This Study Suspension Restart Notification Form authorizes Medidata to restart services previously suspended for the corresponding study indicated below, in accordance with the Study Postponement and Suspension Policy posted at https://www.medidata.com/en/study-postponement-suspension-policy. Except as set forth herein, all terms and conditions of the Agreement between Medidata and Customer or Partner, as applicable (herein collectively referenced to as “Client”) shall apply to this request.

“Anniversary Day” means either: (i) the day following the day of the month of the Single Study SO end date, where the end date is other than 28th – 31st of the month; or (ii) the first day of month, where the Single Study SO end date is 28th- 31st of the month.

Effective upon the Early Study Restart Date set forth herein, Client and its Authorized Users will have access to the study, Medidata will resume providing services and Client will resume paying services fees as set forth in the Sales Order, commencing upon the Early Study Restart Date and for the balance of the revised Sales Order term. For the sake of clarity, the Single Study SO shall reflect a revised end date to account for the Early Study Restart Date.
